Regeneron Pharmaceuticals and Sanofi are increasing discounts on the cholesterol-lowering drug Praluent for Medicare Part D plans, which the companies say will reduce subscribers' monthly out-of-pocket costs to $25 to $150 for the drug. The move comes after Amgen increased discounts on its rival PCSK9 inhibitor, Repatha.

CMS Administrator Seema Verma has granted waivers to states seeking to expand Medicaid eligibility under the Affordable Care Act but impose restrictions not in the ACA. Now some states are developing waiver requests to further expand coverage, add public buy-in options and shift to a single-payer model.
