Nurse reduces readmissions by keeping heart failure patients healthy
Clinical Heart Failure Coordinator Maike Henning is in charge of ensuring heart failure patients at Providence Hospital in Anchorage, Alaska, stay healthy and do not have to be readmitted to the hospital within 30 days of discharge. Henning counsels patients about their condition and shows them how to watch for signs of fluid retention. The program has reduced heart failure readmissions from 17% to less than 11%.
