    The number of accountable care organizations formed by Universal American subsidiary Collaborative Health Systems has reached 31 after it completed ACO deals with 15 groups of doctors.     The Utah Department of Health announced its second data breach in less than 12 months, which has affected about 6,000 Medicaid patients. The breach occurred when a USB device was lost by an employee at third-party contractor Goold Health Systems.
