    Many hurdles need to be cleared before state insurance exchanges are operational. Not least of those is IT interoperability.     The UCLA Health System is enrolling young patients with complex conditions into a medical home to better organize care. The program, part of UCLA's initiative to lower costs by 30% over the next five years, has lowered emergency department visits by more than 50%.
